It's easy and you get it on the spot.

I went to the Poder Judicial in Arequipa. I brought the payment receipt for S/.52.80 after paying the fee at El Banco de la Nacion and had to bring my passport and a passport size photograph. It took less than 10 minutes.

Peru is a nice country to visit for a vacation. There are many nice places, the people are friendly and the food is good but it you shouldn't go there to work for a living. There are better places for that.

146 is a strange amount.

They can afford nice flats, chauffeur driven cars, private security, housemaids,...

I wouldn't get a risky and important medical treatment done in Peru.

You'll better get it done in your homecountry. If you are working then then the health insure which your employer has to paid for by law will foot the bill.
If you aren't working then social services will take care of the bill.
