Visa Certification

General Requirements:

• Form F-006 free
• Receipt of payment for the right to service S/.40.00
• Two (02) front-facing photographs and two (02) passport-size color photographs on a white background
• Simple copy of the applicant’s identity document (1)

Change of Consulate for the Receipt of a Visa

• Form F-007 free
• Receipt of payment for the right to service S/.27.00
• Application expressly indicating the reason for the change and the new consulate
• Simple copy of the applicant’s identity document (1) (beneficiary or guarantor of the visa)
  NOTE: Permission will only be granted once, during the three months following the announcement of the approval of the visa by the Ministry of External Relations

Special Permission to Sign Contracts (tourist and student)

General Requirements:

• Printed Form F-004 free
• Receipt of payment for the right to service S/.27.00
• Valid passport and Andean Immigration Card (TAM) or Boarding and Disembarkment Card (TED)
• Proof of payment (Banco de la Nación) for change of immigration status US$50.00 (Paid upon approval of the form)

Certification of a Copy of a Passport or Foreign Resident Card

General Requirements:

• Printed Form F-002 free
• Receipt of payment for the right to service S/. 13.00
• Proof of the original and two (02) copies of the Foreign Resident Card
• Simple copy of the applicant’s identity document (01)
