Document for Leaving w/Child

Can someone with experience who has a DNI tell me what documents are required to leave Peru with child with and one parent.

A notarized letter from the remaining parent (giving permission) and a passport with the same last name.
I brought my stepson out and I had the letter (Notary), and luckily I had his last name legally changed before we left. The airlines are suspicious should you not have the same last name on the passport. I would suggest bringing a certified birth certificate (your name on it) if your child has a different last name (in the case of single parents).

If the child is using a Peruvian passport then you need a notarized (by Peruvian Notary, not a US notary) letter. This is Peruvian law.

But if the child is using a US passport you need nothing, assuming child has same last name as you. The relevant Peruvian law does not apply to US citizens.
